Pharmacogenetics of TNF inhibitors in rheumatoid arthritis
نویسندگان
چکیده
Etanercept, infliximab and adalimumab have shown clinical benefit in immune-mediated inflammatory diseases; however, the outcome of treatment with these tumour-necrosis factor inhibitors remains insufficient in 40–60% of individuals with rheumatoid arthritis. Moreover, their use is accompanied by adverse events and unintentional immune suppression. Pharmacogenetics has the potential to increase efficacy and ameliorate adverse events and immune suppression, and its application might be of clinical benefit for patients with rheumatoid arthritis. Pharmacogenetic studies have shown associations between single nucleotide polymorphisms in genes encoding enzymes related to the pharmacodynamics and pharmacokinetics of these drugs and treatment outcome. As we discuss here, replication and prospective validation are warranted before pharmacogenetics can be used in clinical practice. 106 Pharmacogenetics of TNF inhibitors in rheumatoid arthritis

Background: Rheumatoid arthritis (RA) is a chronic multisystem autoimmune disease common in all races and ethnics. Cytokines and cytokines receptors play an important role in RA pathogenesis and clinical presentation. Objective: To investigate the serum levels of TNF-α, TNF-α RI, TNF-α RII and IL-12 in RA patients and healthy control group. Methods: In this study 43 patients fulfilling the revi...
متن کاملPredicting response to TNF antagonists in rheumatoid arthritis: the promise of pharmacogenetics research using clinical registries.
Despite the demonstrated efficacy of three different classes of biologic response modifiers (BRMs) for the treatment of rheumatoid arthritis (RA), there are currently no clinical predictors or biomarkers that can rationally guide physicians in the selection of BRMs for individual patients. One promising area of translational research for patients with RA is the field of pharmacogenetics. In the...
متن کاملEffect of TNF-α Blockade in Gingival Crevicular Fluid on Periodontal Condition of Patients with Rheumatoid Arthritis
Background: Periodontitis and rheumatoid arthritis (RA) share a number of clinical and pathologic features, one of which is the presence of the tumor necrosis factor alpha (TNF-α)-induced bone resorption that is involved in the pathogenesis of both. Objectives: To investigate the effect of TNF-α blockade on periodontal conditions in patients with active RA. Method: The periodontal statuses of 3...
